New Client Checklist – Required Documents
Please provide the following documents to ensure accurate and efficient tax preparation.
Personal Information
☐ Photo ID (Driver’s license, state ID, or passport)
☐ Social Security cards (or ITIN letters) for all individuals on the return / SSN is enough if not providing Social Security Cards
☐ Birth dates for you, spouse, and dependents
☐ 3 Years - Prior Tax Returns
Income Documents
☐ W-2 Forms – Wage and salary statements from all employers
☐ 1099 Forms – Including:

☐ Education expenses (1098-T form for tuition, plus receipts for books/supplies)
☐ Student loan interest statement (1098-E)
☐ Medical expenses (Only if significantly high relative to income)
☐ Mortgage interest & property taxes (1098 form)
☐ Charitable contributions (Cash and non-cash donations with receipts)
☐ IRA, 401(k), or HSA contributions (Statements showing contributions made)
☐ Self-employed business expenses (Receipts, mileage logs, home office details, etc.)
Other Financial Information
☐ Estimated tax payments (Dates and amounts of any quarterly payments made)
☐ Foreign bank accounts or income (FBAR filing may be required)
☐ Prior-year tax return (If you’re a new client, bring the last two years)
For Business Owners & Self-Employed
☐ Profit & Loss statement (or a summary of income and expenses)
☐ Business-related purchases & receipts (Equipment, inventory, software, etc.)
☐ Vehicle expenses (Mileage log or total miles driven for business)
☐ Home office expenses (Square footage of office vs. home, utility bills, etc.)
☐ 1099-NEC Forms issued (If you paid independent contractors)
For Real Estate Investors
☐ Rental income & expenses (Breakdown by property)
☐ Mortgage interest (Form 1098) & property tax bills
☐ Depreciation schedules from prior years (if applicable)
For Retirees
☐ IRA/Pension/401(k) distributions (1099-R form)
☐ Social Security benefits statement (SSA-1099)

For Those Who Bought/Sold a Home
